Apply Now
Please read the application carefully and ensure you have all the required documents on letterhead before applying. The application process will take approximately 20 minutes to complete and must be finished once started. If the application does not include the required letterhead documents, the application cannot be processed, and you will need to re-submit the application. See below for suggestions on obtaining documents.
The following official school documents must be on school letterhead and are needed to complete this application:
- Past 12 months of discipline history on school letterhead
- Past 12 months of attendance history on school letterhead
- Past 12 months of grades on school letterhead
- IEP or 504 Plan (if applicable)
Suggestions
-
Contact your child's current school to request the documents above.
-
Another option is to print the report card for 23-24 and 24-25 to show 12 months of attendance and grades. For those that use Skyward and PowerSchool, please note those systems do not show discipline. The family will need to obtain a statement on letterhead from a current school official regarding discipline standing including any and all infractions for the past 12 months.
